I see so many layers between the developer and the CTO that I wonder which is the degree of freedom in this model for the developer, and consequently, the amount of hand-offs necessary between layers, so everything can run smoothly. Plus, if there is any loose on agility due to all the paperwork.

Am I wrong? Or is it just me that I have seen plenty of bad practices? If not, is there any other structure less rigid and more agile?
